NAATP’s Stages of Change Model and Organizational Assessment Tool can assist leaders in treatment and recovery programs in this process. You will need to refer to this Tool as you complete this survey. Using details provided on pages 4-5 in the Tool as a guide, completion of this survey should take about 10-15 minutes.
If your organization does NOT provide clinical services, when the tool refers to “clinical care” or “treatment,” consider instead whatever services or products your organization provides. Instead of “patients,” consider instead the internal and external clients or customers that your organization serves.
Individual responses will be kept confidential, and organization-level summary results will not be shared outside of the organization.
