We are interested in your perspective on how diversity, equity, inclusion & belonging (DEIB) issues are considered in your organization. We want to know your opinions on the work environment (and, if applicable, patient/customer care environment) at your organization, and whether you feel these environments are inclusive and welcoming to all.
This survey is for any employee (part-time, full-time, or contracted) at an addiction or behavioral health organization (or a related field). This includes employees in all positions and roles – administrative, facilities, clinical, support, etc.
All questions are optional, and all responses are kept anonymous. We do not ask for your name. Combined scores and comments from all responses at your organization may be shared with your organization, but individual responses will NOT be linked to demographics, organizational role, or any other potentially identifying information. No comments will be shared with any organization who submits fewer than 3 total staff surveys.
