These addiction healthcare providers are the participants in the FoRSE Addiction Treatment Outcomes Program. Participating providers agree to send a set of de-identified data describing characteristics of the people they serve and the services they provide, as well as patient-reported outcome measures of substance use and mental health symptoms, quality of life, and other aspects of wellness. Providers who are actively sharing data with FoRSE, or FoRSE Data Sites, receive a Data Site seal to indicate their important role in the work of FoRSE. Data Sites receive an annual report from FoRSE, which they can use to compare their services and outcomes to the aggregated data from other participating providers nationwide.
